These would be the gamma incidents detected by S.H.I.E.L.D. according to Marvel Studios Visual Dictionary. I don't think the Hulk is responsible for only six deaths before going to Brazil, since the Hulk is a powerful being facing fragile humans. There are black ops led by Thaddeus Ross that would hide casualties and there would be deaths that could hardly be attributed to the Hulk for lack of evidence.
