Keith Spivey was a nurse at Saint Teresa Center for Mental Health. One day, Spivey helped break out two patients and they found a crypt. Curious, Spivey got closer and ended up getting killed by Basar, who soon took control of his body.

Spivey finds the crypt
Keith Spivey married a woman named Jolene and had two children. He worked at Saint Teresa Center for Mental Health as a nurse in the secure wing. While working with Victoria Helstrom, who had been possessed by the demon Kthara, Kthara drew an ideogram on the wall in Helstrom's blood which convinced Spivey to help stage a breakout. He broke Hock and Liddle out of their cells and left the hospital with the two of them, killing Steven along the way.

Spivey is killed
Spivey, Hock, and Liddle then arrived at a crypt in Colma, California, which they entered together. They went deeper into the crypt, and Spivey became scared when he saw several rats. Spivey, having taken a picture of Kthara's ideogram on his cell phone, looked for it on the walls of the crypt, using the picture as a reference. He found it carved into a casket which housed the Keeper and asked the two for help opening the casket. The two appeared too scared to help, and Spivey expressed confusion about why Kthara had asked those two in particular to help. Spivey began to open it himself, although Hock pushed him into the casket, causing him to bleed. Spivey scolded Hock, although the casket grabbed Spivey and began killing him. Liddle explained that they had been chosen in order to kill Spivey, who then became possessed by Basar.[3]

Basar visits Jolene as Keith
While in Spivey's body, Basar killed Hock and Liddle[3] and began killing several people, including Aubree Richards,[4] and helping his mother, Kthara, escape the psych ward. Spivey's body took severe damage during the escape due to the Keeper's Skull,[2] and the body remained barely alive until finally Kthara removed Basar's heart from it, destroying Spivey's body.[5]

Keith Spivey was very eager to participate in events. While working with Hock and Liddle, he was the only person actively stepping forward and taking charge within the group, suggesting this. He is also very trusting, having incorrectly put his faith in Kthara, Hock, and Liddle without asking too many questions.
